The American Dance Institute Dance School Terms & Conditions

1.0  Assumption of Risk

1.1 I agree to participate/let my child participate in American Dance Institute's classes and hereby release American Dance Institute, its owners, officers, instructors and employees from liability for any injury I/my child may suffer because of my/their participation. 

2.0  Payment Policy

2.1 Tuition (partial or full) must be paid in advance to enroll in any class.
2.2 Please note payment plans require you set up an ACH from your checking account and your total will reflect a 5% administrative fee. 2.3 Credit cards may not be used unless special arrangements have been made with the office. First and last month's tuition will be charged upfront, along with a non-refundable $40 annual registration. Subsequent payments will be withdrawn on the 15th of the month. If you would like a payment plan, please indicate so on the comments section of your registration form,
2.4  30 days notice is required for dropping classes.
2.5  A late fee of $15 will be charged monthly on all past due accounts unless prior arrangements have been made with the office.
2.6 A $35 NSF fee will be charged on all returned checks and returned bank transfers.
2.7  Credit is not provided for missed classes.
2.8  Make up classes are permitted in any age/level appropriate class and must be completed within the same session as the missed class.
2.9  30 days written notice is required to withdraw from a class. Students are responsible for all fees incurred for classes for 30 days after written notice of cancellation is received.
2.10  Medical withdrawals (a serious or long term illness or injury) are eligible for a credit, based on the number of weeks remaining in the session. The credit will be held for one year from the date of withdrawal.
2.11  In most cases, refunds will not be issued to students who withdraw from a class(es).
2.12 I have read and I understand the above requirements as they pertain to my classes.

3.0  Photo Release

31.1 I hereby grant permission to American Dance Institute to publish photographs taken of me, and or my child for use in the ADI's printed publications, promotions, and website.

3.2 I further acknowledge that since my participation in publications and websites produced by ADI is voluntary, I will receive no financial compensation. 

3.3 I further agree that my participation in any publication and website produced by ADI confers upon me no rights of ownership whatsoever.
